Aftercare and relapse prevention
Clinically reviewed by Dr. Abdur Rahim Khan, PhD Psychology (NUMS) · Last reviewed October 2026
Planning for life after discharge, so progress lasts.
The first months after rehab are when relapse is most likely. Before discharge, every patient and family at Health Plus gets a relapse-prevention plan, follow-up appointments and clear guidance on what to do if things start to slip. Completing a stay is only part of recovery. The skills, support and follow-up afterwards matter as much.
